Problem and Issue Report

The purpose of this reporting application is to allow you to submit information on potential or real issues and problems to your managers (particularly "top" managers such as the company Chief Operating Officer or even the Board Chairman). An email is sent to the one or two managers (for whom you provide the email addresses below) from Technidigm.org giving the issue-specific information only (e.g., the location, the topic, and your description/assessment of the issue).

Personal data (your name, etc.) are not sent but may be used on your behalf if needed in the future. While no guarantees can be made, our intention is to provide you with an avenue for raising your concerns in a manner that protects you from possible retaliation.

Keep in mind that, for companies specifically subscribing to this service, the top management has stipulated that they will ensure that those who raise issues in this manner will be protected from retaliation. In most cases, employers provide awards for raising issues or concerns that, when assessed by top managers, are determined to have resulted in significant savings or safety improvements.

Your manager(s) may respond to our email(s) to ask for additional information using the reference number that we assign to your issue. You are also free to inquire or provide updated information.

You may at anytime decide to identify yourself and your concerns and issues to your managers. We would, of course, appreciate receiving notification of this in the form of an updated input, as mentioned above.

Accuracy, completeness, and consistency will improve your credibility, but that is something you have to decide for yourself.
