This form is to be used primarily to SUBMIT information on displaced persons or to ask questions on displaced persons whose location and condition may be of concern to others as a result of natural disasters (eg, Katrina, Rita, and Gustav). You can enter your contact information, any note, question, status, or offer of assistance using any known phone number in the disaster area, whether that phone number is working or not. Whoever checks this database for information on that phone number will read your note and may contact you or add additional information to the database, again based on that phone number. You can check back later to see whether additional information has been added for that phone number.
